Terms of Reference
Organizational Setting and Work Relationships
Medical Section, through the Regional Medical Officer based in Nairobi carried out a mission in June 2022 to carry out an assessment of the Mandatory Health Support Elements in various duty stations in Tanzania. One of the main findings of the mission revealed that in Kibondo there are 277 UNHCR personnel and dependents. Although there is a District Hospital in Kibondo, it is inadequate to address primary care, in-patient hospital care and emergency medical response in case of acute emergencies.

Apart from this, travel from Kibondo to Kasulu and other duty stations is challenging due to the terrain and unpaved roads. Because of this, the main recommendation for Kibondo was to upgrade the UNDP Clinic in the location to ensure capacity for emergency stabilization and emergency response. Hiring a nurse or paramedic skilled in emergency medical response was one of the recommended actions.
Because the nurse will be hired on a UNHCR contract, the technical reporting line will be to the UNHCR Regional Medical Officer in Nairobi while s/he also collaborates with the UN Clinic Doctor based in Kasulu on a day-to-day basis. Administrative supervision will be to the UNHCR Head of Office in Kibondo.
The nurse will be based in the UNHCR Office in Kibondo but is expected to be familiar with the health care providers and medical facilities in the location.

Work implies frequent interaction with the following:
• Staff within the work unit, the UN Clinic, UNHCR Regional Medical Officer and other Medical Section personnel in Nairobi,
• UNHCR Medical Section and Staff Health and Wellbeing Service
• UNHCR personnel in the duty station
• Security personnel
• Staff Counsellors of the UN system
• Humanitarian agencies working with the UN in the duty station
• Health care providers including UN Clinic personnel, UN designated or examining physicians and public, private, governmental and NGO – administered medical facilities.
• External health care providers / suppliers for UN agencies
• Physicians and representatives of Laboratories, Pharmacies

All UNHCR staff members are accountable to perform their duties as reflected in their job description. They do so within their delegated authorities, in line with the regulatory framework of UNHCR which includes the UN Charter, UN Staff Regulations and Rules, UNHCR Policies and Administrative Instructions as well as relevant accountability frameworks. In addition, staff members are required to discharge their responsibilities in a manner consistent with the core, functional, cross-functional and managerial competencies and UNHCR’s core values of professionalism, integrity and respect for diversity.

Duties
– Clinical:
– Provide primary Health Care services for UNHCR personnel and eligible dependents. This includes:
• Carry out clinical assessment of patients.
• Acute care of common medical conditions and the management of acute exacerbations of chronic health problems, according to clinical guidelines.
• Facilitate access or referral to diagnostic laboratory and imaging facilities.
• Coordinate medical care with UN Clinic Doctor in Kasulu.
• Facilitates referrals to medical specialists in the duty station or in-country referral location.

– Assists UNHCR RMO / UN Clinic Doctor in providing health education and health promotion programs; provide advice and / or lead in carrying out preventative health measures include health advice, relating to:
• Malaria and Dengue prevention.
• Food and water hygiene.
• First responder training.
• COVID-19

– Respond to emergency calls and provide Pre-Hospital Emergency Care (PHEC) 24/7. It includes immediate response, resuscitation, and stabilization of life-threatening emergencies. This may require facilitation of transport of patients to existing district hospital, liaison with doctors and nurses in the Emergency Department, Ward or Intensive Care Unit and referral to the Regional Medical Officer in cases where MEDEVAC is required.

– Participates in work environment assessment, e.g. office ergonomics, and addresses other occupational health issues.
– Patient advocacy
– Coordinates medical evacuation and cooperates with UNHCR RMO in ensuring patients receive immediate care while awaiting transport for MEDEVAC.
– Assists in providing medical support during evacuation and repatriation.
– Advises evacuees on the requirements for evacuation and/or escorts patients if needed.
– Maintain strict confidentiality with regards to patient’s medical records, patient management and treatment.
– Organize the UNHCR Clinic in the location and ensure compliance with UN healthcare quality and patient safety standards.
– Assist RMO with Health Facility Assessments and drafting medical referral pathways for the duty station.
– Participate in Health Sector Coordination meetings.
Administrative:
– Maintain the Clinic in good order and keep the inventory of medical supplies and equipment up-to-date and be prepared for emergencies or disasters.
– In consultation with the UN medical physician, maintain a 24-hour roster of available specialists for referral and treatment in the duty station and capital city.
– Check equipment and supplies on a daily, weekly, monthly and yearly basis and maintain them in working order.
– Ensures that medical instruments, devices are properly sterilized and maintains hygiene and cleanliness of the clinic.
– Through a monthly inventory ensures that medical instruments and equipment in the UN ambulance are maintained in a working order, and replaced after use. Work with Admin /Supply to ensure replenishment of medications, consumables and supplies.
– Enter medical data of patients into the patient’s file in the medical database.
– Schedule consultations with the UN Physician for eligible staff and dependents;
– Ensure a proper and safe disposal of hazardous material and medical waste in close coordination with the UN Doctor
– Drafts monthly clinic reports.
– Perform other related duties as required.

Minimum Qualifications
Education & Professional Work Experience
Years of Experience / Degree Level
• For G6 – 3 years relevant experience with High School Diploma; or 2 years relevant work experience with Bachelor or equivalent or higher
• Registered Professional Nurse and first level degree from an accredited Baccalaureate
• Nursing Programme (University) or equivalent such as an accredited Diploma Programme (4 years).
• Certificates in ECG, CPR or Basic Life Support and ACLS or equivalent emergency medical care is an asset.
• Recognized additional training in primary health care and midwifery is desirable;
Field(s) of Education
• Nursing Health Care Management, or other relevant field.

Certificates and/or Licenses
• First Aid Training Basic Life Support Pre-Hospital Trauma Life Support
HIV/AIDS Management training is desirable.
National nursing registration and license is a requirement.(Certificates and Licenses marked with an asterisk* are essential)
Relevant Job Experience
Essential
• Minimum 5 years of continuous clinical experience in nursing preferably in a multicultural environment;
• Intensive Care and health administration. Supervisory experience in a UN field operation is an asset.
Desirable
• UN field experience in the occupational health care sector is desirable.
• Previous experience in humanitarian crisis and infectious diseases is highly desirable.
• Knowledge and understanding of relevant UN administrative policies and procedures is desirable.
• Experience in Trauma and Emergency care is desirable
• Experience in the usage of computers and office software packages (MS Word, Excel, etc.) is desirable.
Functional Skills
• MD-UN-wide health care policies and protocols (e.g. medical evacuation procedures, etc.)
• MD-Assessment of quality standards of health facilities
• MD – Conduct health risk assessments
• PH-Public Health Surveillance, Outbreak Investigation and Response
• LE-Data protection laws & procedures for sectoral practices (employment, health, security)
• OS-Occupational Health and Safety critical risk control methods
• OS-Occupational Health and Safety statistical reporting
• MD-UN Medical Service standards and practices
• MD-Medical Evacuation route planning and operationalization with MOU
