2. Job Tittle: Medical Social Worker
Duty station: Moshi Kilimanjaro Reports to: Health and Training Officer.

Duties and Responsibilities.
A Medical Social Worker at Faraja Forward will be responsible for the following duties;
• Conduct assessment and evaluating client’s social conditions to determine service required.
• Conduct complete case management of each child (client) case. Conduct monitoring and follow ups for proper and quality service delivery.
• Ensure proper admission procedures of children into database and update accordingly.
• Guiding patients and families in understanding the illnesses, conditions, and treatment options
• Performing psychosocial interventions with individual children and families.
• Coordinate referral of client cases that might need specialized services to partner health care providers to ensure continuity of care for children and their families.
• Educating families and community on issues of physical, mental, and emotional health
• Assisting patients with medications, medical devices, transportation, and advance directives.
• Support the Health and Training Officer in delivering health trainings to families of children with special needs.
• Any other duties as may be assigned by supervisor.

Education/Experience/Language requirements
• Degree or advanced diploma in social work, Diakonia or its equivalent.
• At least 2years experience in clinical social work practice
• Passionate in working with children with special needs.
• Practical experience in community outreach work is required
• Knowledge in casement and psychosocial support and counselling is required.
• Good command in English and Kiswahili language.
