Helen Keller International (HKI) is a nonprofit organization dedicated to saving and improving the sight and lives of the world's vulnerable by combatting the causes and consequences of blindness, poor health, and malnutrition. We currently have more than 120 programs in 20 countries around the world including Tanzania.

The HKI-Tanzania office was opened in 1984 and was originally located in Dodoma. Our office has since moved to Dar es Salaam and works in close partnership with the Ministry of Health, (MoH), local NGOs, and other national and international development partners in the areas of nutrition, maternal and child health, and neglected tropical diseases.

HKI- Tanzania is currently implementing programs that are aimed at combatting the causes and consequences of blindness, poor health, and malnutrition by strengthening and increasing access to and utilization of education on maternal health, nutrition, and trachoma disease. Currently, the programs are implemented at Dar es Salaam, Pwani, Lindi, Mtwara, Mara, Mwanza, Tanga, Ruvuma, Katavi, Arusha, Manyara, Singida, Dodoma, Morogoro, Njombe, Mbeya, Iringa, and Songwe.

Helen Keller International (HKI) seeks to invite interested and eligible companies and individuals to submit applications for the pre-qualification of suppliers from the above-mentioned regions.

DETAILS FOR SUBMISSIONS OF THE PRE-QUALIFICATION DOCUMENTS

The pre-qualification documents must be organized in a PDF file and must be clearly named a name of the company/individual with the category applied for and sent through email to Tz.procurement@hki.org with the title "SUPPLIERS PRE-QUALIFICATION FY2024" Please indicate clearly the appropriate Reference Number of goods/service category you are applied for, and submit by 31s December 2023

PRE-QUALIFICATION INSTRUCTIONS
1. Helen Keller International herein referred to as the organization, requests applicants/candidates who meet the criteria set out by the organization to apply for registration of pre-qualification.

2. The pre-qualification objective is to supply and/or deliver assorted items and also provide services under the relevant tenders/quotations to Helen Keller International on and as when required during the stated period.

3. Duly registered suppliers of goods and services under the Laws of the United Republic of Tanzania are invited to submit their legal and statutory documents to Helen Keller International so that they may be pre-qualified for submission of quotations or proposals. The prospective suppliers are required to supply mandatory information for pre-qualification.

4. Prospective suppliers and contractors must have carried out successful supply and delivery of similar items/services to NGOs/Corporations/Institutions/Governments of similar size. Potential suppliers/contractors must demonstrate the willingness and commitment to meet the pre-qualification criteria.

5. This invitation of application for pre-qualification is open to potential applicants who are able to demonstrate proven experience, and technical, financial, and managerial capacity to supply the listed goods and services.

6. This document includes questionnaire forms and documents required of prospective suppliers.

7. The successful applicants will be registered on the Helen Keller International suppliers list for the period Feb 2024 - to Feb 2025 or MORE as seem fit and the organization will only deal with the firms that are registered.

8. Applicants must submit a duly completed and signed Confidential Business Questionnaire provided herein as Attachment 'A.

9. Helen Keller International reserves the right to request the submission of additional information from applicants or any other credible source and to visit and inspect the business premises of the applicant to verify the information given.

10. The organization will ensure the information that is received from companies is treated with the utmost confidentiality and shall be for the sole use of the organization.

11. The pre-qualified suppliers are required to immediately advise the organization of any significant change in its financial, technical capacity, ownership, or holdings it may have.

12. Failure to submit any of the mandatory requirements will lead to automatic disqualification from the exercise.

ESSENTIAL CRITERIA FOR PRE-QUALIFICATION
1. Experience
A. Prospective Bidders shall have at least five (5) years of experience in the supply of goods or services applied for.
B. Prospective suppliers require special experience and capability to organize the supply and delivery of items or services at short notice.
C. Helen Keller International reserves the right to request additional qualification information as the tender/quotation stage to suit particular procurement.
D. You are required to share contracts of previous works done in the past five years at least one from each year.
E. No recommendation letters will be accepted or considered as references. Helen Keller International may request for written email or video conference with reference if that may seem fit.

Financial Capability
A. The supplier's financial capability will be determined by the latest financial statement submitted with the pre-qualification document as well as the latest contracts performed by the respective bidder accompanied by letters of reference from their bankers/clients regarding suppliers/contractors' credit positions.
B. Potential suppliers/contractors will be pre-qualified on the satisfactory information given.
A. Special consideration will be given to the financial capability to execute orders.

3. Past Performance
B. Past performance will be given due consideration for any pre-qualifying bidders.
C. Share previous contract/LPO alongside delivery note/good received a note to prove that goods or services were delivered as agreed.
D. Share the copies of the business license of the 4 previous years alongside with current business license.

4. Premise
A. The Firm must have a registered and fixed business premise in Tanzania mainland, with a Certificate of Registration.
C. Share any documents showing the evidence of physical registered office premises.
D. Helen Keller International reserves the right to do due diligence and visit the premises physically.

5. Statutory Obligations
A. The firm must show proof that it has paid statutory obligations and has a valid Tax compliance certificate from the Tanzania Revenue Authority (TRA).
B. The vendor must attach the relevant copies of business licenses.

PRE-QUALIFICATION EVALUATION CRITERIA

6. Financial Strength
A. Provide Official Stamped Bank statements for the last three (3) months.
B. Provide the latest 3 contracts performed along with their signed GRN/Delivery notes

7. Delivery & Logistics:
A. State location of registered office, warehouse, and space whether owned/rented indicating:
• Building
• Street/Road
• City/Town
B. Submit a brief statement of supply and service delivery methods and procedures the firm
proposes/plans to use to execute the contract.

8. State whether your company is a:
A. Manufacturer
B. Authorized agent (Attach principal/manufacturer's authorization letter)
A. Other (Please specify)

9. Specify the amount of business you can handle at any one time in value of Tanzania Shillings (TZS)

10. Payment terms: Agree (& YE) Dis-agree (@ NO)
A. Payment will be made within 30 days after delivery upon Helen Keller Intl inspection and acceptance and receipt of vendor invoice.
B. All payments shall be made to supplier/contractor by wire transfer from Helen Keller Intl to supplier/contractor
C. Under no circumstances, will HKI issue payment to an individual or company other than the supplier/contractor

11. Give any other information that you feel is relevant for purpose of Pre-qualification as a supplier.
